PositionOverview: The Service Coordinator/Case Manager will support family needs andcoordinate internally and externally to help eligible families’ enrollment intoNYS Early Intervention Program and follow up on the whole process with properdocumentation and within approved timeline. Location: Manhattan,NY Language:Bilingual English & Chinese KeyResponsibilities: Provide information and guidance to families who need EarlyIntervention Services for their children Provide assistance and coordinate actively in the arrangementprocess of an IFSP meeting (Individualized Family Service Plan) Regular and effective communication with families during homevisits to follow up on the case implementation process and understand familyneeds, priorities and concerns Maintain proper documentation of each child case to meet thestandards of regulation requirements and ensure accurate and timely submissionin the system Work closely with therapists and internal teams in anefficient and effective manner Key Requirements: Bachelor degree in Psychology, Public Health, Education,Social Science or any other human services related field Bi-lingual in Chinese or Russian Excellent communication and interpersonal skill Well organized and be able to manage multitasks in anefficient and effective manner Self-motivated, flexible and work independently withinitiatives under minimum supervision Excellent written skill with paperwork and all sorts ofdocuments Proficient computer skill
